HeartMap Institute began with a simple observation:
too many patients with chronic conditions were falling through the cracks between appointments.
As a Nurse Practitioner, Valerie saw firsthand how many patients struggled with:
- fluctuating blood pressure
- uncontrolled diabetes
- unmanaged heart conditions
- frequent hospital visits
- lack of daily support
- confusion about their health
At the same time, clinics were overwhelmed, short-staffed, and unable to check on patients consistently.
That’s where the idea for HeartMap Institute was born.
